Dermal Filler
-
Dermal fillers offer a versatile, non-surgical solution for addressing a range of cosmetic concerns, including reducing deep lines and wrinkles, filling hollows, and smoothing facial contours. They are also effective in adding or restoring volume to areas such as the temples, under-eyes, cheeks, nose, lips, chin and jawline, which can provide definition, enhance facial symmetry and improve overall appearance.
As we reach our mid-twenties, the body's production of collagen and elastin, proteins vital for maintaining firm, elastic skin, begins to decline. This reduction, coupled with the gradual breakdown of existing collagen, leads to common signs of aging, such as sunken cheeks, hollow eyes, deep lines, and wrinkles. Without sufficient collagen and elastin, the skin loses its strength and resilience, making it more prone to sagging and wrinkling.
Dermal fillers effectively combat these signs of aging by replenishing lost volume, smoothing out lines and harmonising facial features, resulting in a naturally youthful and rejuvenated appearance. These treatments are popular for their ability to deliver noticeable, long-lasting results without the need for invasive surgery.

Most dermal fillers on the market are composed of hyaluronic acid, a naturally occurring molecule found in all skin and soft tissues throughout the body. Hyaluronic acid is consistent across species and plays a crucial role in skin health by attracting and retaining water, which provides volume and hydration. It also supports collagen and elastin fibers, offering structural 'cushioning,' and acts as a carrier for chemical signals between cells. When used in dermal fillers, hyaluronic acid is formulated into a clear, sterile gel that is injected into the skin, delivering immediate and noticeable corrections.
These dermal fillers are typically manufactured through a process called cross-linking, which involves bonding the hyaluronic acid molecules with proteins to create a stable gel suitable for injection. In the UK, there are over 160 brands of dermal fillers on the market, partly due to less stringent regulations. However, it’s important to note that not all hyaluronic acid fillers are created equal, and reputable brands are not available for general purchase online. They should only be administered by trained and qualified clinicians to ensure safety and effectiveness.
Each reputable brand offers a range of tailored products designed to achieve specific aesthetic outcomes, as one formulation does not fit all needs. Leading and well-established brands in the industry include, but are not limited to:
Juvederm™ (FDA approved in 2006)
Restylane™ (FDA approved in 2003)
Belotero™ (FDA approved in 2011)
Teosyal™
These brands have built their reputation on quality, safety, and efficacy, making them the preferred choices for clinicians and patients alike.

The results of dermal fillers are typically visible immediately after treatment. The product continues to hydrate and integrate into the tissue over time, offering a slight increase in volume enhancement as it settles. Some initial bruising or swelling is expected, which generally subsides within 1 to 2 weeks.
Dermal filler is not permanent since it is gradually broken down and absorbed by the body over time. Depending on the specific type of filler and the treatment area, results can last anywhere from 6 to 18 months. Your medical practitioner will recommend the most suitable type of dermal filler based on your needs and desired outcomes.

Certain medical conditions and medications can influence the decision to proceed with treatment or may require adjustments to the treatment plan. To ensure safe and effective results, it is essential to conduct a comprehensive medical history review and a thorough assessment of your expectations before making any decisions about treatment.

Treatment with dermal filler may not be recommended if any of the following apply:
You have an active skin infection in or near the treatment area, or if you are unwell in any way, including having a cold.

You are taking medications that affect blood clotting, such as aspirin, warfarin, or other anticoagulants.
You are on steroids or have an autoimmune disease.
You have any bleeding disorders.
You have a poorly controlled medical condition, such as diabetes.
You are pregnant or breastfeeding.
You are currently taking or have recently completed a course of Roaccutane (Isotretinoin) within the last 18 months.

Your clinician will provide aftercare advice to minimise risk and promote speedy recovery from expected side effects.
Most individuals can resume their normal activities immediately, with noticeable improvements in their appearance. However, some may experience mild redness, small pin-prick marks, and potential swelling, especially in the lips.
Bruising is also possible and should subside within a few days. Swelling and bruising may be more noticeable the day after treatment; applying cold compresses to the area can help reduce these effects.
Makeup should not be worn for 24 hours.
For the first 24 hours after your procedure, you should also avoid alcohol, vigorous exercise, sunbathing, and exposure to extreme temperatures.
If you received a topical anaesthetic cream on the treated area, particularly on the lips, exercise caution with hot food and drinks for 12 hours post-treatment, as you may have temporary numbness.
-
All treatments come with a degree of risk, which will be thoroughly discussed with you during your consultation and before any treatment is agreed upon. To ensure safety and effectiveness, dermal fillers should only be administered by a healthcare professional with specialised training.
Possible side effects include, but are not limited to:
Redness
Swelling
Bruising
Itching
Tenderness
It's important to note that these side effects are typically mild and temporary. They often resolve within the first few days, with swelling usually subsiding within hours. Bruising, if it occurs, can take up to a week to 10 days to fade, though in some cases, it may persist slightly longer. Rest assured, these side effects are part of the normal healing process and should not cause undue concern.

Most leading dermal filler brands include a local anaesthetic within the filler itself to minimise discomfort during the procedure. Additionally, clinicians may apply topical anaesthetic cream to the treatment area beforehand to further numb the skin. As a result, the treatment is generally well-tolerated and should not be painful.

The longevity of dermal fillers can vary depending on the specific product used and individual factors. Generally, most fillers last between 6 and 18 months.
